Case Manager – Family & Supportive Services
Location: New York, NY (on-site)
Position Overview
We are seeking a dedicated and compassionate Case Manager to join our Family and Educational Support Services team. The Case Manager will provide comprehensive case management and advocacy to families, ensuring access to housing assistance, entitlements, and educational resources. This role requires strong interpersonal skills, problem-solving ability, and the capacity to engage families from diverse backgrounds in building stability and self-sufficiency.
Key Responsibilities
Conduct intake and needs assessments to develop individualized service plans with measurable goals.
Provide direct case management services related to housing stability, including rental arrears, housing repairs, permanent housing opportunities, and eviction prevention.
Collaborate with the Educational Advocate to support children and youth with school enrollment, advocacy, and access to educational resources.
Advocate for families with schools, landlords, city agencies, and community partners to ensure access to entitlements and supportive services.
Conduct regular case reviews, home visits, and progress evaluations to identify evolving family needs.
Provide crisis intervention, conflict resolution, and referrals to external providers when necessary.
Maintain consistent communication with families regarding benefits, entitlements, workshops, and community events.
Develop and co-facilitate workshops on housing stability, financial literacy, parenting support, and family empowerment.
Attend school meetings, parent/teacher conferences, and community events to support family engagement.
Maintain accurate and timely case records, data entry, and reporting in databases and spreadsheets.
Prepare monthly reports and contribute to program evaluation, communications, and administrative functions.
Support community-building efforts that foster participation in BHC programs and services.
Perform other duties as assigned.
Qualifications & Skills
Bachelor’s degree in Social Work, Human Services, Education, or related field required; Master’s degree preferred.
Minimum 2 years of case management, housing advocacy, or family support experience.
Knowledge of NYC housing programs, entitlements, and public school systems strongly preferred.
Knowledge of NYC and NYS agencies.
Excellent communication, advocacy, and conflict-resolution skills.
Ability to conduct crisis intervention with calmness, empathy, and professionalism.
Strong organizational and time-management skills with ability to manage multiple priorities.
Proficient in maintaining accurate documentation and case records in digital systems.
Demonstrated ability to work independently and collaboratively in a multidisciplinary team.
Cultural competence and experience working with families from diverse backgrounds.
Bilingual (Spanish/English) strongly encouraged to apply.

Salary commensurate with experience and the position will pay $60,000-$65,000 per year. We offer a comprehensive benefits package, including health, dental, vision, 403(b) retirement plan, group life insurance, short term disability, long term disability and paid time off.
